Medication Lock Boxes

Safeguard your medicine and take extra precautions by participating in our free medication lock box program.

Students and employees can receive free medication lock boxes at the Student Health Center (SHC) pharmacy. Just stop by the pharmacy, complete a short request form, and get your lock box at that time.

Lock Box Request Forms

Option 1 – In person (paper form): Ask the SHC pharmacy staff for a copy to complete when you visit.

Option 2 – Online (survey): Complete the request form online before your visit, then let the SHC pharmacy staff know you’ve already submitted the survey.
